ACCESS TO CARE

Hawaiʻi’s communities are diverse and geographically dispersed. Many patients face transportation challenges, workforce shortages, language differences, and other structural barriers. Expanding access requires thoughtful system design, strong partnerships, and a commitment to accessible health care. Part of improving access is ensuring that individuals and families across Hawaiʻi can receive timely, comprehensive, high-quality primary care.

STRENGTHENING THE SAFETY NET TOGETHER

The Hawaiʻi Primary Care Association partners with our 14 Federally Qualified Health Centers (FQHCs) to strengthen the systems that make this possible. Through targeted training and technical assistance, we help health centers enhance how care is delivered, coordinated, and sustained so patients can access the services they need. By doing so, we strengthen the entire safety net. That means more coordinated services, improved quality, and better health outcomes statewide.

We partner with health centers to strengthen the systems that support access to care. This includes:

  • Refining scheduling, referral, and care coordination processes

  • Supporting team-based approaches to care

  • Advancing integration of primary care, behavioral health, and enabling services

  • Exploring and enhancing telehealth and other technology-enabled access strategies

  • Strengthening language access and patient communication efforts

  • Supporting outreach and engagement with underserved communities

Our work centers on practical, operational improvements. We assess needs, facilitate shared learning across health centers, and provide tools and approaches that can be adapted within local settings.
