How we Do this work
We support community health centers across Hawaiʻi through a combination of direct support, shared learning, and statewide coordination. Our approach is designed to be practical, responsive, and adaptable, allowing us to meet health centers where they are and adjust as needs change.
Our work includes individual training and technical assistance, learning teams, in-person convenings, webinars, and learning opportunities delivered through our learning management system, along with ongoing communication through our digest and other activities that support planning, implementation, and continuous improvement. By offering multiple ways to engage, we help health centers build capacity, strengthen connections, and stay informed across a wide range of focus areas.
Together, these efforts allow us to provide consistent support while remaining flexible and responsive to the evolving needs of our community health center network.

Health Center Performance Improvement Framework
The Performance Improvement Framework guides how we support community health centers in strengthening systems, operations, and care delivery over time. It provides a shared structure for understanding where health centers are today and identifying opportunities for growth that align with their goals and capacity.
We are currently exploring this framework to better understand how our training, technical assistance, and shared learning efforts can align across programs and partnerships. At this stage, we are listening, learning, and identifying opportunities for stronger coordination statewide. This process allows us to thoughtfully assess fit and consider how we can build a more connected, intentional approach over time.
